Fix incorrect placement of Ansible provisioner in Vagrantfile. Update Ansible playbook to create swarm based on learnings from Interop ITX Swarm demo. Change machines.yml to use bento/ubuntu-14.04 instead of ubuntu/trusty64. Unable to test due to slowness/connectivity issues to Ubuntu repositories.
