d6a55ee31c Add '?max_comments=n' query parameter 
This increases the initial max-comments-to-download above the user's
preference without changing their saved setting. This can be useful when
linking to a specific comment which is new enough that the default max-
comments setting (of 800) is too low to download the linked comment
tree, causing most users to see a confusing 'No comments found' message.

aac22ce109 Speed up and clarify the 'load more comments' UI 
* only rerender the comments after loading and iff something changes
 * change text to 'loading...' and make link unclickable while waiting
 * if all comments are already loaded, rename to 'load new comments'

6d1600ff01 Use a token bucket to rate-limit the Pushshift API 
Queries to Pushshift will now be started about as quickly as
permitted, while also reducing 429 responses to a minimum.

c98f71bd4d Rewrite api/pushsift/index.js using async/await 
* much simpler, got rid of recursion
 * no more Set temporaries, less copying of arrays
 * now returns a Map (thread/index.js need not create it)

2f4c8e0c62 Work around 8-year-old Firefox bug 
If the max comments setting was changed using only the spinner, it
wasn't saved. See https://bugzilla.mozilla.org/show_bug.cgi?id=1012818

fe21a0760f Improve speed of changing max comments setting 
* Just changing it no longer causes all comments to be rerendered
 * Reloading now only reloads comments intead of entire page
 * Reload button correctly hides when new setting <= current

3230ebc0d8 Stop using OAuth2 with the Reddit API 
None of the API endpoints being used require authentication,
its use was slowing things down a bit for no advantage.

854f56e6f2 Fix bug where status is sometimes stuck "loading" 
This was introduced in 9e343fc. There may still be a race condition
here, however it should occur far less frequently than before.
